What are Guedel Airways used for?

The Guedel Airway, commonly referred to as an oropharyngeal airway, is a critical device for airway management. It's designed to prevent the tongue from obstructing the airway, which is vital for patients who are unconscious or have limited control over airway reflexes. Here are the key uses:

  • Airway Obstruction: It's deployed to prevent the tongue from blocking the epiglottis, which is essential for patients with a diminished Glasgow Coma Scale (GCS)
  • Resuscitation: This airway is integral to ensure a clear airway during resuscitation efforts.
  • Anaesthesia: Utilised to maintain an unobstructed oropharyngeal airway in patients under general anaesthesia, both throughout the procedure and in recovery.
  • Ventilation Support: It can be used in conjunction with oxygen masks or bag-mask ventilation to secure a clear passage for oxygen.

How to use Guedel Airways

  1. Preparation: First, confirm the patient's mouth is clear of any foreign objects.
  2. Lubrication: Apply lubricant to the oropharyngeal airway to facilitate a smooth entry.
  3. Selection of Size: Measure from the patient's first incisors to the angle of their jaw to determine the appropriate size of the airway.
  4. Insertion: Gently insert the airway with the tip pointing upwards towards the roof of the mouth to avoid depressing the tongue and causing an obstruction.
  5. Rotation: Once the tip reaches the back of the throat, rotate it 180 degrees to secure the tongue and complete insertion.
  6. Positioning: The airway should be advanced until the flange rests outside the patient's lips. Be careful not to pinch the lips between the airway and the teeth.
  7. Patient Response: Watch for a gag reflex. If the patient can't tolerate the airway, cease insertion and consider an alternative, like a nasopharyngeal airway.
